Unlike muscle or joint pain, neuralgia often appears suddenly, can be very intense, and may last seconds, minutes, or continuously throughout the day.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full is-resized\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"1024\" src=\"https:\/\/neurohirurgija.in.rs\/wp-content\/uploads\/2025\/08\/ChatGPT-Image-Aug-24-2025-08_14_37-AM.png\" alt=\"Pain in the face\" class=\"wp-image-5128\" style=\"width:586px;height:auto\"\/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>How Nerve Pain Starts (Simple Mechanism)<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Neuralgias can be <strong>secondary<\/strong> when they are caused by a <strong>primary illness<\/strong> such as a surrounding tumor, infection, disc herniation, or similar conditions, or they can be <strong>primary<\/strong>, which are typically caused by less evident causes that are often not visible on scans but are diagnosed through a <strong>physical examination<\/strong>. Here, we will only cover <strong>primary neuralgias<\/strong>. Primary neuralgias are the most common type and usually respond well to targeted treatment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Neuralgia appears when a nerve is:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>irritated<\/strong> (inflammation, tension, nearby swelling),<\/li>\n\n\n\n<li><strong>compressed \/ entrapped<\/strong> (muscle spasm, scar tissue, tight anatomical tunnels),<\/li>\n\n\n\n<li><strong>injured<\/strong> (after surgery, trauma, shingles),<\/li>\n\n\n\n<li>or affected by <strong>neuropathy<\/strong> (diabetes, toxins, vitamin deficiency, metabolic problems).<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The causes of neuralgia are often combined. If they persist for a long time, they eventually lead to damage to the nerve itself (neuropathy), and at that point, in addition to occasional breakthroughs of pain, the pain begins to have a constant, lasting component.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Typical Characteristics of Neuralgic Pain<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">People often describe neuralgia as:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>stabbing or shooting pain,<\/li>\n\n\n\n<li>electricity or burning,<\/li>\n\n\n\n<li>hypersensitivity to touch, wind, or shower water,<\/li>\n\n\n\n<li>pain triggered by speaking, chewing, bending, sitting, or walking (depending on the nerve and local muscles that irritate the nerve),<\/li>\n\n\n\n<li>pain that follows a <strong>line<\/strong> or <strong>specific zone<\/strong> rather than a wide area.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Neuralgia is usually unilateral (one side) and follows a predictable nerve pathway.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Diagnosis of Neuralgia<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">For most patients, diagnosis is made through:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>clinical examination<\/strong>,<\/li>\n\n\n\n<li>targeted <strong>MRI or CT<\/strong> mostly to exclude secondary neuralgia<\/li>\n\n\n\n<li>laboratory tests when metabolic neuropathy is suspected.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">A good clinical history is often more important than imaging.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>General Treatment Principles<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Treatment depends on the cause, but common approaches include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>nerve-specific medications<\/strong> (gabapentinoids, tricyclics, selected anticonvulsants),<\/li>\n\n\n\n<li><strong>reducing mechanical triggers<\/strong> (posture, ergonomics, avoiding pressure),<\/li>\n\n\n\n<li><strong>treating the underlying cause<\/strong> (disc surgery, cyst treatment, decompression),<\/li>\n\n\n\n<li><strong>nerve blocks or targeted injections<\/strong>,<\/li>\n\n\n\n<li><strong>physiotherapy<\/strong> for entrapment syndromes,<\/li>\n\n\n\n<li><strong>surgery<\/strong> only in selected cases when conservative therapy fails.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Most neuralgias improve with correct diagnosis and tailored treatment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Pain in neuralgia usually continues because&nbsp;<strong>not all factors that irritate the nerve have been properly identified<\/strong>&nbsp;and treated. As a result, even when patients received appropriate single or several medications, the therapy remained incomplete. Once all contributing sources of irritation are recognized,&nbsp;<strong>most cases improve only with a targeted combination of medications<\/strong>&nbsp;that addresses every underlying mechanism \u2014 supported by vitamin or supplement therapy. If pain still persists,&nbsp;<strong>ultrasound-guided nerve blocks<\/strong>&nbsp;or, in rare cases,&nbsp;<strong>surgical decompression<\/strong>&nbsp;may be considered.
