Because the intercostal nerve is long and travels through several layers of muscles, fascia, and rib articulations, irritation may occur at different points along its path and still produce the <em>same characteristic chest-wall pain<\/em>. These local irritators can cause pain on their own (\u224810\u201320%) or, more commonly, coexist with another underlying cause such as a rib\u2013spine joint dysfunction, postherpetic neuralgia or postoperative scarring. Treatment failure often happens because attention is focused on only one suspected location (e.g., the costovertebral joint), while other structures along the nerve\u2019s course \u2014 muscle spasm, fascial adhesions, or rib mechanics \u2014 continue to irritate the nerve. In most prolonged cases, pain persists not because the nerve cannot heal, but because the actual sites of irritation along its pathway were never identified and treated.<\/li>\n\n\n\n<li><strong><a href=\"https:\/\/neurohirurgija.in.rs\/en\/postherpetic-neuralgia\/\">Postherpetic neuralgia (PHN)<\/a><\/strong> after shingles (sometimes even without visible skin changes) along a thoracic dermatome \u2014 <strong>\u224845\u201355%<\/strong>.<\/li>\n\n\n\n<li><strong>Minor trauma or rib fracture<\/strong>, even when not detected on X-ray \u2014 <strong>\u224810\u201315%<\/strong>.<\/li>\n\n\n\n<li><strong>Thoracic disc herniation or foraminal stenosis<\/strong> compressing the nerve root \u2014 <strong>\u22485\u201310%<\/strong>.<\/li>\n\n\n\n<li><a href=\"https:\/\/neurohirurgija.in.rs\/en\/postsurgical-pain-syndromes\/\"><strong>Post-surgical pain<\/strong>,<\/a> especially after thoracotomy, mastectomy, or chest drain insertion \u2014 <strong>\u224820\u201325%<\/strong> (of which 5\u201310% remain severe and persistent beyond 6 months).<\/li>\n\n\n\n<li><strong>Tumors, infections, or metabolic neuropathies<\/strong> (such as diabetes) \u2014 <strong>&lt;5%<\/strong> (rare).<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">In some patients, the pain cycle is maintained <strong>long-term<\/strong> by both <strong>nerve hypersensitivity<\/strong> and <strong>secondary irritants<\/strong> (such as inflammation and muscle tension).
