This cavity is filled with <a href=\"https:\/\/neurohirurgija.in.rs\/en\/ventricular-system-cerebrospinal-fluid-csf-simple-explanation\/\">cerebrospinal fluid (CSF)<\/a> and can slowly expand over time.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Unlike tumors or infections, a syrinx does not destroy tissue directly. Instead, symptoms arise because the expanding cavity causes mechanical compression, disrupting the function of the spinal cord and its neural pathways. In its early stages, the damage caused by a syrinx is functional and potentially reversible. Over time, this cavity may enlarge or remain stable, depending on the underlying cause.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This explains why syringomyelia is often associated with structural abnormalities that disturb CSF circulation rather than primary spinal cord disease.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 id=\"causes\" class=\"wp-block-heading\">Common causes of syringomyelia<\/h2>\n\n\n\n<h3 id=\"chiari\" class=\"wp-block-heading\"><a href=\"https:\/\/neurohirurgija.in.rs\/en\/chiari-malformation-adults\/\">Chiari malformation type I<\/a><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The most frequent cause in adults. Downward displacement of the cerebellar tonsils interferes with CSF flow at the cranio-cervical junction. In intramedullary spinal cord tumors, the syrinx most commonly involves the central canal or the adjacent spinal cord tissue, and despite not always being confined to the central canal, it is still classified as syringomyelia.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Arachnoid adhesions and inflammation<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Scarring after infection, surgery, or hemorrhage may disrupt CSF circulation.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Idiopathic syringomyelia<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">In some patients, no clear cause is identified despite detailed imaging.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 id=\"location-size\" class=\"wp-block-heading\">Location, size, and number of involved spinal segments<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Syringomyelia most commonly affects the <strong>cervical spinal cord<\/strong>, but its location, length, and extent vary significantly between patients.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Cervical syringomyelia<\/strong> is the most frequent form, present in approximately <strong>60\u201370%<\/strong> of adult cases, often associated with Chiari malformation type I.<\/li>\n\n\n\n<li><strong>Cervicothoracic syringomyelia<\/strong> accounts for about <strong>20\u201330%<\/strong> of cases and may extend over multiple vertebral levels.<\/li>\n\n\n\n<li><strong>Isolated thoracic syringomyelia<\/strong> is less common, occurring in approximately <strong>5\u201310%<\/strong> of patients, often related to trauma, tumors, or arachnoid pathology.<\/li>\n\n\n\n<li><strong>Holocord syringomyelia<\/strong>, extending through most of the spinal cord, is rare and typically associated with long-standing cerebrospinal fluid flow obstruction.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Size and length of the syrinx<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The <strong>absolute diameter of the syrinx does not reliably predict symptom severity<\/strong>.<br>Small syrinxes may be symptomatic if strategically located, while larger cavities may remain clinically silent.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In terms of length:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>many syrinxes span <strong>3\u20135 vertebral segments<\/strong>,<\/li>\n\n\n\n<li>others may extend over <strong>more than 10 segments<\/strong>, especially in long-standing or post-traumatic cases.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Clinical relevance depends more on <strong>progression over time<\/strong> than on size at a single MRI examination.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Number of syrinx cavities<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">In most patients, syringomyelia presents as <strong>a single continuous cavity<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">However:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>multiple syrinx cavities<\/strong> or <strong>segmental syringomyelia<\/strong> can occur, particularly in post-traumatic cases or in the presence of arachnoid adhesions,<\/li>\n\n\n\n<li>these cavities may be separated by segments of apparently normal spinal cord.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The presence of more than one syrinx does not automatically indicate a more severe condition, but it does require careful imaging review and follow-up.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 id=\"symptoms\" class=\"wp-block-heading\">Symptoms of syringomyelia<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Symptoms depend more on <strong>functional impact<\/strong> than on the absolute size of the syrinx.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Common symptoms include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>chronic or neuropathic pain, often described as burning, stabbing, or electric, which may involve the neck, shoulders, arms, trunk, or legs depending on syrinx location<\/li>\n\n\n\n<li><strong>Cape-like loss of pain and temperature sensation<\/strong>, affecting the neck, shoulders, upper back, and proximal upper limbs, typically seen with cervical syrinx involvement.<\/li>\n\n\n\n<li>neuropathic pain or burning sensations in the arms or legs, depending on syrinx level<\/li>\n\n\n\n<li>muscle weakness or stiffness, most often affecting the hands and arms in cervical syringomyelia<\/li>\n\n\n\n<li>hand clumsiness or gait instability<\/li>\n\n\n\n<li>sensory asymmetry between the left and right side of the body<\/li>\n\n\n\n<li>in advanced cases, bladder or bowel dysfunction<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Some patients remain completely asymptomatic for years, even with a visible syrinx.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Important clinical note<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Not all neurological symptoms in patients with syringomyelia are necessarily caused by the syrinx itself.<br><strong>Coexisting spinal, neurological, or peripheral nerve disorders are common and must be carefully
