They are not caused by lifestyle, stress, trauma, or high blood pressure, and many remain silent for years.<\/li>\n\n    <li><strong>Unlike a brain AVM, a brain or CNS cavernoma has no large feeding arteries or draining veins that directly enter the lesion.<\/strong> Blood flow is low-pressure and occurs through small capillary-type channels.<\/li>\n\n    <li><strong>The main risks of a brain or CNS cavernoma are seizures and small internal bleeding.<\/strong> Most brain cavernomas cause irritation of nearby brain tissue from repeated micro-bleeds rather than massive hemorrhage. However, clinically significant bleeding is most concerning when a cavernoma is located in the brainstem, spinal cord, or near critical brain centers, where even small hemorrhages can cause noticeable neurological deficits.<\/li>\n\n    <li><strong>About 30\u201350% of brain or CNS cavernomas never cause symptoms<\/strong> and are discovered incidentally on MRI.<\/li>\n\n    <li><strong>MRI is the key test for brain and CNS cavernoma diagnosis.<\/strong> Cavernomas have a characteristic \u201cpopcorn\u201d or \u201cmulberry\u201d appearance with a dark hemosiderin rim from prior micro-bleeding. Blood flow is present within a cavernoma and occurs slowly through these abnormal capillary-like spaces. Blood enters via small arterial capillaries and exits via venous capillaries, which connect to the normal arterial and venous circulation of the surrounding brain. Unlike AVMs, cavernomas do not have a direct arteriovenous shunt or large, identifiable feeding arteries and draining veins. Because there is no high-pressure arterial inflow, small leaks and repeated micro-bleeds are more common than massive hemorrhages. Population studies suggest that <strong>about 0.4\u20130.8% of people<\/strong> have a cavernoma, and many are discovered <strong>incidentally<\/strong> when MRI is performed for unrelated reasons. <\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 id=\"multiplicity\" class=\"wp-block-heading\"><strong>Multiplicity and Inheritance (Single vs Multiple CNS Cavernomas)<\/strong><\/h2>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Sporadic Cavernoma<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Most cavernomas are <strong>congenital<\/strong>, meaning they are present at birth, but some can develop later in life or appear de novo, particularly in familial cases or after brain radiation. <strong>In most cases<\/strong>, cavernomas are <strong>not inherited and occur sporadically.<\/strong> <\/p>\n\n\n\n<h3 id=\"inherited-form\" class=\"wp-block-heading\"><strong>Familial Cavernoma (Inherited Form)<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">In <strong>some cases<\/strong>, the condition is <strong>hereditary (genetic)<\/strong>. Over time, this leads not only to chemical irritation but also to local inflammation and the formation of gliotic (scar-like) tissue around the lesion. This chronically altered brain environment can become electrically unstable and often results in seizures, especially when the cavernoma is located in the cerebral cortex.<\/li>\n\n\n\n<li><strong>Symptomatic hemorrhage:<\/strong> A more significant bleed that causes a neurological deficit. This is especially concerning in <strong>eloquent brain regions<\/strong> (critical areas controlling speech, movement, or vision) or the <strong>brainstem<\/strong> and <strong>spinal cord<\/strong>, where even a small amount of pressure can impact vital functions.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 id=\"symptoms\" class=\"wp-block-heading\"><strong>Clinical Presentation (Symptoms)<\/strong> of <strong><strong>Brain or CNS<\/strong><\/strong> Cavernoma<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Approximately <strong>30\u201350%<\/strong> of cavernomas remain <strong>asymptomatic throughout life.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Common presentations include:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Seizures<\/strong> (especially with supratentorial cavernomas) in <strong>~25\u201340%<\/strong> of patients<\/li>\n\n\n\n<li><strong>Focal neurological symptoms<\/strong> (weakness, numbness, speech or balance problems), in <strong>~10\u201325%<\/strong> of patients, depending on location. These are caused by repeated microbleeding, local edema, and venous congestion around the cavernoma \u2014 even without a large hemorrhage<\/li>\n\n\n\n<li><strong>Headache<\/strong> (nonspecific) in <strong>~10\u201320%<\/strong>. In many cases, the headache is caused by another condition rather than the cavernoma itself, such as migraine or <a href=\"https:\/\/neurohirurgija.in.rs\/en\/occipital-neuralgia-and-occipital-headache\/\">occipital neuralgia<\/a>, which should be properly identified and treated.<\/li>\n\n\n\n<li><strong>Symptomatic hemorrhage:<\/strong> Hemorrhage is less common as an initial presentation compared with seizures.
